 The Ministry of Evangelization & Catechesis supports parents in their role as primary
 educators of their children by offering Faith Formation for children in grades K-8 so they
 can discover the joy of our Catholic Faith. All elementary & middle school children not
 attending St. Brendan School are expected to register for our program beginning in first
 grade, however we encourage parents to enroll their child beginning in Kindergarten!

Instructions Please complete this registration form and enter ALL fields accurately - this information will be used for all Faith Formation communication & will help keep our parish database current.
AGREEMENTS: By completing this form, I understand that unless I contact the Faith Formation office otherwise, I agree to the following: 1.) I am the legal guardian & have custodial right to enroll my child/children in this program. 2.) My child/children may be transported & receive medical care if the Faith Formation staff is unable to reach me in the event of an emergency during campus monthly gatherings. 3.) I release my child/children's photo to be used for the St. Brendan website, bulletin & other social media to promote the Faith Formation program.

Once you complete the information for your first child, you will have the option to register another child or submit the completed registration. You will receive a verification e-mail of your submitted registration & a separate e-mail within a week that includes the link to make your payment. Once you receive the link, please make your payment within 7 days unless other arrangements are made with the Faith Formation office.
